Full Time Job Summary:
Serves as Nurse practitioner for the clinic, including patient education, diagnosis and treatment of illness under medical protocols. Referral of patients for appropriate follow up care. Accountable and receives direction from Medical Director. Operates under medical protocols established by the Board of Administrators and the medical director. Collaborates with physician as necessary for prescriptive privileges. Responsibilities
  • Teaching, counseling, diagnosing and treatment of illness under protocol for chronic care (Shalom) and/or acute care visits when applicable.
  • Sustaining and supporting patients during diagnosis and treatment.
  • Obtaining health history and assessing health/illness status.
  • Maintaining quality assurance of patient care given or assigned.
  • Collaborative approach to healthcare utilizing a care team method with required monthly meetings
  • Providing patient education in the areas of health promotion and disease prevention.
  • Referral of patients as necessary and appropriate to other health providers and/or social service team
  • Appropriate follow up of patients with acute and chronic health problems as indicated by the Medical Director and Data Analyst for outcome measures to be tracked
  • Other duties emanating from the Medical Director and/or of the Board Directors or as assigned by Executive Director.
  • Stays current with the policies of the clinic and other in house departments as necessary to provide an integrated and holistic approach to our patient population
  • Stays current with healthcare protocols with collaborative physician in line with current medical practice and reports to Medical Director on needed changes in protocols.
  • Willing to participate in our Wellness program for education/fitness classes designed to improve the health outcomes of our patient population
  • Willing to participate in our outside events for fundraising, community awareness, and/or patient engagement
  • Charting, refills, and telephones encounters are completed in a timely manner
  • Other duties as assigned
  • Skills and Knowledge Required
  • Thorough knowledge of the expanding role of a Nurse Practitioner.
  • General knowledge of quality assurance practices and procedures.
  • Skills in interpersonal, community and group relations.
  • Ability to exercise sound leadership and judgment.
  • Technical skills to function as a medical provider.
Education and Experience Minimum of a Bachelor's degree in nursing and advance preparation and certification by the American Nurses Association as a nurse practitioner. Master's degree preparation and ANA certification as an advance practice nurse
