Job Description

Our healthcare organization serves multiple skilled nursing and rehabilitation communities in South Texas is seeking a compassionate and experienced Nurse Practitioner to join its clinical leadership team. This role provides medical support across several post-acute care facilities within a regional service area and offers the opportunity to make a meaningful impact on the health and well-being of older adults. Position Summary The Nurse Practitioner will provide comprehensive primary and post-acute care services to residents in skilled nursing and rehabilitation settings. This position partners closely with interdisciplinary teams to deliver high-quality, patient-centered care while promoting positive clinical outcomes and enhancing quality of life. Key Responsibilities Conduct comprehensive patient assessments, including physical examinations and review of medical histories. Diagnose, treat, and manage acute and chronic health conditions. Develop and implement individualized care plans. Order, interpret, and evaluate diagnostic tests and laboratory results. Prescribe medications and treatments in accordance with applicable regulations and clinical guidelines. Collaborate with physicians, nursing staff, therapists, and other healthcare professionals to coordinate care. Support care transitions, discharge planning, and follow-up services. Educate residents and family members regarding treatment plans, disease management, and wellness strategies. Maintain accurate and timely documentation within electronic medical record systems. Participate in quality improvement initiatives and clinical review processes. Qualifications Current Nurse Practitioner license and prescriptive authority in the State of Texas. Experience providing care to geriatric populations, preferably within skilled nursing, rehabilitation, long-term care, or post-acute settings. Strong assessment, diagnostic, and clinical decision-making skills. Proficiency with electronic medical records and clinical documentation. Excellent communication and relationship-building abilities. Ability to work independently while collaborating effectively within a multidisciplinary team.
