Modern office setting Growth opportunities Wellness programs Job Overview We are seeking an advanced practice provider(NP/PA-C) to join our healthcare team. The ideal candidate will have a strong background in outpatient primary care and possess a diverse skill set to provide high-quality patient care. This role is designed to grow into a Service Line Lead (Wound Care) with direct influence on clinical operations and workflow design. Responsibilities
  • Provide advanced outpatient clinical Internal Medicine and Wound Care in a multidimensional clinic setting. 30% Primary Care and 70% Wound Care mix.
  • Utilize eClinicalWorks electronic health record systems for accurate documentation and patient care coordination.
  • Conduct health coaching sessions to promote wellness and disease prevention. Evaluate and manage Primary Care conditions such as, but not limited to, diabetes, hypertension, asthma, coronary disease, hyperlipidemia, osteoarthritis and mental health disorders.
  • Perform triage assessments to prioritize patient needs effectively.
  • Administer treatments including advance wound care therapies, general dermatology, Texas Compassionate Use Program, weight loss management, allergy testing and other specialized procedures as indicated under physician supervision. Qualifications
  • Must hold a valid Physician Assistant or Nurse Practitioner license.
  • Expert level in using electronic health record systems for documentation.
  • Strong communication skills and ability to work within a multidisciplinary team.
This position offers the opportunity to work in a multidimensional Internal Medicine practice, providing a challenging yet rewarding experience.
Pay:
$107,000.00
  • $130,000.
