In this role, you will deliver comprehensive primary care across diverse patient populations, including adults, geriatrics, and individuals with complex medical needs. Your expertise will empower patients to achieve optimal health outcomes through thorough assessments, personalized treatment plans, and patient education. Duties Conduct detailed physical examinations and health assessments for patients, utilizing your extensive physiology and anatomy knowledge. Develop, implement, and monitor individualized care plans that incorporate evidence-based practices across primary care and geriatrics. Utilize eClinicalWorks for accurate medical documentation and coding (CPT, ICD-10). Provide triage services via telehealth or in-person to prioritize urgent patient needs while coordinating referrals for specialty services like cardiovascular, podiatry, gastroenterology, infectious disease care, or hospice & palliative medicine. Experience Proven experience as a Family Nurse Practitioner or in similar advanced practice nursing roles within outpatient clinic settings. Hands-on experience with eClinicalWorks is essential for efficient documentation. Knowledge of CPT coding and ICD coding standards ensures accurate billing and documentation for insurance purposes. Most importantly, providing the best care possible to each and every patient as if he/she is family.
