Job Description

POSITION SUMMARY
At Vital Heart & Vein, our specialists provide a full spectrum of diagnostic testing, comprehensive therapies, and interventional/electrophysiologic procedures utilizing the latest technology. We are seeking an experienced Cardiology Advanced Practice Provider (APP) to join our team in a hybrid inpatient and outpatient role , combining hospital-based rounding responsibilities with clinic-based patient care. This position will be primarily based out of our Pearland and Southeast clinics, with responsibilities across affiliated hospital locations.
POSITION RESPONSIBILITIES
Outpatient /
Clinic Responsibilities:
Provide outpatient for cardiac patients managing conditions such as hypertension, hyperlipidemia, coronary artery disease, valvular disease, arrhythmias, and heart failure under the supervision of a physician Conduct pre- and post-imaging, intervention, and surgical follow-up visits in the clinic Perform comprehensive assessments and develop individualized, evidence-based treatment plans Communicate plan of care with patients and provide patient education and other pertinent resources accordingly Integrate current clinical guidelines and evidence-based practices to optimize patient outcomes Collaborate with physicians, nurses, and other staff to coordinate patient care efficiently Inpatient /
Hospital Responsibilities:
Perform hospital rounding on cardiology patients, including new consults and follow-ups Evaluate and manage acute and chronic cardiovascular conditions in the inpatient setting Collaborate with attending physicians, hospitalists, and multidisciplinary teams on patient care plans Assist with care transitions, including admissions, discharges, and coordination of post-hospital follow-up Provide patient and family education regarding diagnoses, treatment plans, and post-discharge care
General Responsibilities:
Maintain timely and accurate documentation in the electronic medical record (eCW) Support organizational goals through high-quality patient care, efficiency, and professionalism Participate in clinical team meetings and contribute to continuous improvement efforts Perform other job-related duties as assigned, with the understanding that responsibilities may be adjusted to meet organizational needs Conduct pre- and post-imaging, intervention, and surgical follow-up visits in the clinic Perform comprehensive assessments and develop individualized, evidence-based treatment plans Integrate current clinical guidelines and evidence-based practices to optimize patient outcomes Collaborate with physicians and clinic staff to coordinate patient care efficiently
EDUCATION & LICENSURE
Required:
Advanced Practice Registered Nurse (FNP-C, AGACNP-BC, or
ACNPC-AG
) or Certified Physician Assistant (PA-C) license Must be licensed to practice as an Advanced Practice Registered Nurse (APRN) or Physician Assistant in the state of Texas
EXPERIENCE
Required:
A minimum of two (2) years of experience as an Advanced Practice Provider in adult cardiology
Preferred:
Experience in both inpatient (hospital-based) and outpatient cardiology settings
KNOWLEDGE, SKILLS, & ABILITIES
Strong clinical assessment, documentation, leadership, and analytical skills Ability to manage patient care across both inpatient and outpatient settings Effective collaboration with multidisciplinary teams in both hospital and clinic environments Exceptional communication skills with patients, families, physicians, and colleagues Strong critical thinking, organizational, and problem-solving abilities Ability to work autonomously with minimal supervision in a dynamic, fast-paced environment Efficient navigation of Electronic Health Record (EHR) systems and workflows for both clinic and hospital settings
