Our mission is simple; to bring compassionate, evidence-based care directly to the patient's bedside while partnering with the community to optimize outcomes. We are seeking a dedicated and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our interdisciplinary clinical team. Key Responsibilities Provide services to patients on home health services, skilled nursing facilities, assisted living, and clinic based settings. Perform physical exams and medical history assessments Diagnose and treat chronic and acute health conditions including pain management Order and interpret diagnostic tests and lab results Develop and adjust individualized care plans Prescribe medications and monitor treatment effectiveness Collaborate with facility staff and interdisciplinary teams Educate patients and families about treatment options and care goals Ensure a safe, respectful, and supportive environment for residents Qualifications Licensed Nurse Practitioner or Physician Assistant in the State of Texas (required). Active DEA license (or eligibility to obtain). Minimum 1 year of NP/PA experience preferred Strong clinical judgment, communication, and documentation skills.
