HCHC HCHC
is a non-profit 501(c)3 federally qualified health center, a true universal access medical facility. HCHC brings together an exceptional team of health care providers, a responsive and helpful administrative staff and a supportive Board of Directors to fulfill our mission of providing comprehensive primary care health services to anyone in the community regardless of their financial situation. Position Summary The role of NP at HCHC is to provide high quality primary health care patient services to support the Mission and Vision of HCHC. The NP will support HCHC's organizational values and strategic goals including care delivery thru our patient centered medical home model. The NP will support the HCHC Quality Improvement plan and goals including initiatives to support patient safety, the patient experience, access to care, and initiatives to improve the health of the community. The NP will actively participate in HCHC's Peer Review process. Responsibilities Provides comprehensive primary care within the collaborative agreement documents scope of practice defined by HCHC privileging protocol for the medical specialty. Care will be delivered in full accordance with HCHC policies and protocols, including: Provide wellness, chronic disease, same day, and emergency services in accordance with the patient's needs. Provide a clear assessment and safe treatment plan for each patient encounter that takes into account the patient's circumstances. Order, perform, and interpret routine laboratory and diagnostic imaging tests and notify patients of results per HCHC protocol. Provide access to primary care thru Telehealth and phone. Support wellness and restoration through self-care, continuity of care and care coordination. Refer patients for specialized care based upon an identified need. Incorporate recommendations from consultants into treatment plans. Complete documentation in the EMR per HCHC protocols. Comply with EMR maintenance and upgrade activities. Collaborates with physicians and clinical team in accordance with collaborative agreement documents to plan and coordinate patient care. Provides professional expertise for in-service training and Quality Improvement projects. Actively participates in Quality Improvement initiatives and Peer Review Obtains and maintains appropriate privileges at area community hospitals. Attends internal and external meetings, workshops, conferences, and participates in task forces or committees as necessary and/or required by HCHC. Demonstrates commitment to HCHC's recognition as a Patient Centered Medical Home and its associated excellence standards by modelling excellence in all clinical, administrative and team process relationships, addressing service excellence deficits. Provides call coverage, cross coverage, Saturday clinic coverage and other shared work. Other duties as assigned. Required Skills and Abilities Must be able to prioritize, have cognitive reasoning, able to mathematically calculate for drug therapy. Must be aware of diversity among patient's ethnicity and the impact of patient's cultural background. Education and Experience Current and Unrestricted Family Nurse Practitioner license to practice medicine in the Commonwealth of Virginia. DEA license to prescribe medicine. Board certification in chosen field of practice. Cannot be sanctioned under Medicaid or Medicare. Maintains federal, state and professional licenses/qualifications to maintain scope of practice in Virginia. Current CPR (BLS) certificate is required. Previous experience working in a Community Health Center environment is preferred but not required. Physical Requirements Hearing must be adequate to perform the essential functions of the job such as: auscultating blood pressure, ability to hear soft voices. Speaking must be adequate to perform the essential functions of the job, such as: Clearly communicating instructions, providing input on assessments and evaluations of patients, taking medical history, relaying information to multiple personnel, patients and their families. This is a moderately active role, which requires the ability to lift or move up to 25 pounds, bending, squatting, sitting and standing as necessary. Must be able to lift, carry and handle equipment, supplies and other work site materials based on duty requirements. Work Location You may be required to travel to any of our sites as part of your responsibilities at any time.
