Job Description

Job Overview We are seeking a dynamic and compassionate Nurse Practitioner to join our healthcare team. In this vital role, you will deliver high-quality patient care across diverse settings, in our Family Medical Practice environment. Your expertise will help improve patient outcomes through comprehensive assessments, diagnosis, treatment planning, and ongoing management. Responsibilities Conduct thorough physical examinations and detailed patient assessments to determine health status and identify medical issues. Develop and implement individualized treatment plans, including prescribing medications and therapies such as botulinum toxin treatments or dermal fillers when appropriate. Manage acute and chronic conditions across various specialties such as acute care and chronic conditions Utilize electronic health record (EHR) systems like Athena to document patient encounters accurately while adhering to HIPAA regulations. Provide patient education on health coaching, medication administration, nutrition, and disease prevention strategies. Participate in telehealth consultations to extend care access and support patients remotely across diverse populations including assisted living residents or individuals with developmental disabilities. Experience Proven experience working in family medicine and urgent care clinics. Demonstrated proficiency with EMR/EHR systems like Athenahealth for medical documentation and coding (CPT, ICD-9/10). Strong background in managing complex cases involving chronic care conditions Experience performing diagnostic evaluations using laboratory testing or medical imaging techniques; familiarity with sonography is a plus. Prior work with diverse patient groups Ability to work independently to deliver exceptional patient-centered care.
