In this role, you will provide expert symptom management, facilitate advance care planning conversations, and offer emotional support to the patient and family experiencing chronic illness in an acute hospital setting. You will serve as a vital bridge between the patient and the broader healthcare system, ensuring care aligns with the patient's personal values. Key Responsibilities •
Clinical Assessment:
Conduct comprehensive physical, psychosocial, and spiritual assessments for patients with complex medical conditions who are admitted to a community hospital center. •
Symptom Management:
Diagnose and treat distressing symptoms such as pain, nausea, dyspnea, fatigue, and anxiety using pharmacological and non-pharmacological interventions. •
Care Planning:
Develop and implement individualized plans of care in collaboration with the patient, family, and other healthcare team members. •
Advance Care Planning:
Lead difficult discussions regarding illness trajectory, treatment options, and care preferences. •
Interdisciplinary Collaboration:
Work closely with referring providers, consultants and hospital system and assist to bridge patient to appropriate community programs for continued support. •
Documentation:
Maintain accurate, timely electronic health records (EHR) in compliance with regulatory standards and billing requirements. Qualifications & Skills •
Education:
Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program. •
Licensure:
Active Nurse Practitioner (NP) license in the state of Virginia •
Certification:
Board certification in a specialty (e.g., Adult-Gerontology, Family, or Acute Care); Board Certification in Hospice and Palliative Care (ACHPN) is strongly preferred. •
Experience:
Prior clinical experience in palliative care, hospice, oncology, geriatrics, or intensive care preferred. Minimally 1 year of NP experience required. Work Environment •
Settings:
Community Hospital in Fairfax County •
Schedule:
Typically Monday-Friday, 9:00AM to 5:00 PM, with potential call responsibility with compensation. Benefits •
