Compensation:
$98,000 - $135,000
Annual Salary Benefits:

Position Description:
We are looking for a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) for our Skilled Nursing Facility ( SNF ) in Barre, VT for bedside patient care. The provider serves as the primary medical contact for assigned residents, partnering closely with the Medical Director, facility nursing leadership, and interdisciplinary teams. This is a full-time, in-person position with predictable weekday hours and strong work-life balance.
Qualifications:
Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Vermont, in good standing Active DEA registration 1+ years of clinical experience; SNF, geriatrics, or primary care preferred Strong clinical documentation skills and familiarity with EMR systems Excellent communication skills with patients, families, and interdisciplinary teams
Responsibilities:
Upon admission, examine residents and review their medical records to establish baseline assessments Conduct routine follow-up visits and manage chronic conditions including diabetes, hypertension, CHF, COPD, and dementia Prescribe and adjust medications including controlled substances per state regulations Lead transitional care management and discharge planning with social work, nursing, and rehabilitation teams Engage with residents' families on care plans, goals of care, and advance directives If you're a skilled and compassionate NP or PA looking for a challenging and rewarding opportunity with our Skilled Nursing Facility ( SNF ), we want YOU to apply!
