Days Schedule:
4 10-hour days- Minimum of two years' experience as a primary care provider preferred

- Conducts comprehensive assessments, diagnose conditions, develop plans, and prescribe medications for patients across the lifespan
- Diagnoses and treats acute and chronic disease using evidence-based guidelines
- Provides preventive care, health education and counseling to individuals and families
- Timely and accurate documentation, inclusive of direct patient encounters and asynchronous care
- Participates in the coverage for clinic colleagues per departmental guidance
- Participates in after-hours call coverage per departmental guidance Multidisciplinary Collaboration
- Participates in multidisciplinary meetings, care reviews and care planning
- Coordinates care with all members of the delivery team
- Contributes to the development and improvement of population-based care Population Health Management
- Utilizes data and risk stratification tools to prioritize our patients at greatest risk to prioritize outreach
- Engages in quality improvement focused on health outcomes, cost effectiveness and cost reduction
- Supports transitions of care and reduced readmissions and ED use Citizenship
- Demonstrates the MultiCare values of compassion, stewardship, kindness, joy collaboration and respect
- Adheres to MultiCare policies and processes Requirements
- Maintains active Washington State Nurse Practitioner or Physician Assistant license
- DEA, and state prescriptive authority
- Current BLS for Healthcare Providers certification by the American Heart Association.
