  • Manage complex patients, reduce hospitalizations, and improve health outcomes What You'll Do
  • Provide mobile primary care to geriatric patients within 25 miles of your home
  • Manage chronic conditions ( CHF, hypertension, diabetes, etc. ) to enhance quality of life
  • Collaborate with a healthcare team to ensure consistent, high-quality patient care What We're Looking For ✔ Licensed NP/PA-C in Washington ✔ DEA License (or ability to obtain within 90 days) ✔ BLS/CPR Certification ✔ Passion for geriatric care and chronic disease management ✔ Reliable transportation & home office setup
