This Family Practice Clinic Provider position for a Physician Assistant or ARNP joins an award-winning rural health organization known for exceptional quality, patient experience, and strong community partnership. The Advanced Practice Provider benefits from flexible scheduling, a modern EHR, telehealth expansion, and data-driven care pathways. The Family Medicine Nurse Practitioner or PA-C serves pediatric through geriatric populations with compassionate, evidence-based Primary Care. Candidates for this Advanced Practice Clinician role bring 3+ years of Primary Care experience with board certification or eligibility and an active state license.
Aligned with a Critical Access Hospital, this Primary Care Advanced Practice Provider works in an outpatient family practice setting that ensures seamless transitions of care. The Family Practice Provider maintains a full-time 40-hour schedule with 32 patient-facing hours and 8 administrative hours for coordination and documentation. The Physician Assistant or ARNP typically manages about 15 patients per day to preserve access and quality. Integrated services empower the Advanced Practice Provider with on-site Behavioral Health and Dental resources for comprehensive, whole-person care.
Reporting to the Clinic Director and partnering with a Medical Director, this Family Medicine Advanced Practice Provider is supported by friendly nurses, skilled MAs, pharmacists, care coordinators, and Behavioral Health colleagues. The Physician Assistant or ARNP evaluates and treats acute and chronic conditions, advances patient education for chronic disease management, and contributes to population health initiatives. The Primary Care Clinician participates in quality improvement, uses metrics to drive outcomes, and supports process redesign. In collaboration with leadership, the Advanced Practice Clinician provides oversight for new employees or students, fostering a learning culture.
