- Part-time We are a growing, physician-owned medical group expanding in Mequon, WI, seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in an Inpatient Rehab Facility (IRF) setting.
This role focuses on post-acute and rehabilitative care, including orthopedic recovery, stroke management, and pain management, working alongside a collaborative lead provider.
Key Responsibilities:
Provide comprehensive medical care to IRF patients, including assessments, diagnosis, and treatment planning Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions Ensure accurate and timely documentation in
EMR Required Qualifications:
Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Wisconsin Active DEA license (or ability to obtain prior to start) Minimum 1 year of clinical experience as an APP (SNF, IRF, geriatrics, internal medicine, or rehab experience strongly preferred) Experience managing adult/geriatric patient populations Strong clinical decision-making and time management skills Comfortable working independently in an IRF setting
Preferred Experience:
Skilled Nursing Facility (SNF) or post-acute care experience Background in internal medicine, geriatrics, or physical medicine & rehabilitation (PM&R) Experience with orthopedic, stroke, or pain management patients
Schedule & Setting:
Part-Time, 3 days per week No nights, no weekends "Round and go" model for strong work-life balance Inpatient Rehab Facility setting

Physical Requirements:
Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer. Must be able to lift up to 50 pounds. May be exposed to bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards. Must wear the required PPE per company and/or facility requirements. Must comply with facility vaccination requirements.
