This position focuses on providing high-quality primary care services within geriatric and long-term care settings, with an emphasis on patient-centered care, prevention, and improving quality of life. We are looking for a provider who values meaningful patient relationships, collaboration, and the opportunity to grow within a supportive and mission-driven organization. What You'll Do Conduct patient assessments, physical exams, and routine follow-up visits Diagnose and manage acute and chronic health conditions, including diabetes, hypertension, and asthma Complete admission history and physicals Perform consult and discharge medication reconciliation Provide preventive care and patient education focused on wellness and disease prevention Collaborate with nursing staff and interdisciplinary care teams Support patients with behavioral health needs through compassionate, whole-person care Qualifications Master's or Doctorate degree in Nursing Current Family Nurse Practitioner certification Active Wisconsin Nurse Practitioner license Strong communication and clinical decision-making skills Experience in primary care, geriatrics, or long-term care preferred, but motivated new providers are encouraged to apply Why Join Sequoia?
