Strive Health is a growing mental health practice dedicated to delivering compassionate, high-quality psychiatric care to residents in skilled nursing and long-term care facilities. We believe every patient deserves access to expert mental health support - and every clinician deserves the resources to deliver their best care.
Job Summary:
We are seeking a full-time Nurse Practitioner to provide psychiatric evaluations, medication management, and follow-up care for residents in nursing homes and assisted living settings. You'll be supported by a full-time medical director, administrative staff, and outsourced billing services - so you can focus on patient care.
Responsibilities:
Conduct initial psychiatric assessments and routine follow-up visits Prescribe and manage psychotropic medications Collaborate with facility staff, medical directors, and care teams Document in EMR in a timely and accurate manner

Nurse Practitioner Job Qualifications:
Must have a current and active NP license and DEA # in the State of Wisconsin. Must have at least one year experience working as a Nurse Practitioner. Be willing and able to commute to long term care facilities. Experience with geriatric populations or interest in working with older adults Compassionate, patient-centered care philosophy Strong communication and collaboration skills

Full-time Pay :
Competitive base + productivity bonus (commensurate with experience)
Schedule :
Monday to Friday (flexible hours)
Work Setting:
On-site at skilled nursing/long-term care facilities
