We provide internal medicine and primary care in skilled nursing facilities, rehab centers, and senior living communities.

As we continue to expand, we are looking for Nurse Practitioners and Physician Assistants to join our growing team in post-acute care. Why This Role? You'll have the ability to manage your day, build relationships with patients, and make a direct impact on their recovery and quality of life. You'll work alongside physicians and interdisciplinary teams, with the support and structure needed to succeed without burnout. Position Highlights Full-time or part-time availability No nights, weekends, or call "Round and Go" model - manage your schedule and patient load independently Provide care in SNFs, rehab centers, and senior living communities Collaborate with physicians, nurses, and facility staff Manage new admissions, follow-ups, and chronic care for geriatric patients Participate in care coordination and quality improvement initiatives What We're Looking For Nurse Practitioner (FNP, AGNP) or Physician Assistant (PA-C) Active, unrestricted Alabama license DEA registration (or ability to obtain) Strong clinical and decision-making skills Ability to work independently in a post-acute setting Experience in geriatrics, internal medicine, or post-acute care preferred Comfortable using EMR systems Strong communication and teamwork skills Compassionate, reliable, and patient-focused What You'll Do Perform patient assessments, exams, and follow-ups Manage acute and chronic conditions in a post-acute population Complete admissions, progress notes, and discharges Coordinate care with physicians and facility staff Ensure accurate, timely documentation in EMR Support care transitions and reduce hospital readmissions Contribute to facility quality and compliance initiatives What We Offer Earning potential based on encounters Flexible scheduling Liability insurance coverage Clinical and operational support (scribes, coders, dashboards, compliance tools) Ongoing training, mentorship, and professional growth opportunities Collaborative, physician-led team environment Work Locations Skilled Nursing Facilities (SNFs) Inpatient Rehab Facilities (IRFs)
