  • 5:00 PM Horizon Post Acute & Rehabilitation is hiring an experienced MDS Nurse (RN or LPN) to join our clinical reimbursement team.
We are looking for a nurse with at least one year of MDS experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting who is comfortable working with MDS assessments, RAI documentation, Care Area Assessments (CAA's), and interdisciplinary care plans . This role works directly with the MDS Coordinator and interdisciplinary team (IDT) to ensure accurate Minimum Data Set (MDS) completion, care planning, and documentation that supports quality care and proper reimbursement under Medicare and PDPM guidelines . Responsibilities Assist with completion and coordination of Minimum Data Set (MDS) assessments Support the Resident Assessment Instrument (RAI) process Complete and review Care Area Assessments (CAA's) Develop and update interdisciplinary care plans Ensure timely MDS submissions in compliance with federal and state regulations Work closely with nursing, therapy, and leadership as part of the Interdisciplinary Team (IDT) Identify resident acuity, needs, and services required Maintain documentation supporting Medicare, PDPM, and skilled services Qualifications Active RN or LPN license in Arizona Minimum 1 year MDS experience in a Skilled Nursing Facility (SNF) or Long-Term Care Working knowledge of: MDS assessments RAI process Care Area Assessments (CAA's) Care planning PDPM / Medicare documentation Strong attention to detail and organization Ability to work collaboratively with IDT teams What We Offer Competitive pay based on MDS experience Health, dental, and vision insurance Paid time off Retirement plan options Professional development opportunities Supportive leadership team and strong clinical culture At Horizon Post Acute, we are committed to providing exceptional care while creating a workplace where nurses can build long-term careers.
