In this vital role, you will provide high-quality, patient-centered care to individuals across various settings, including hospitals, long-term care facilities, assisted living communities, and home care environments. Responsibilities Assist patients and residents with activities of daily living such as bathing, dressing, grooming, mobility, and feeding to promote independence and comfort. Monitor and record vital signs including blood pressure, temperature, pulse, and respiration rates accurately using electronic health record (EHR) systems. Support medical procedures by preparing patients for treatments like catheter care, phlebotomy, or medication administration under supervision. Maintain a safe environment by using equipment such as Hoyer lifts and ensuring infection control practices like aseptic technique are followed diligently. Document patient information thoroughly in medical records while adhering to HIPAA regulations to ensure confidentiality. Collaborate with healthcare teams in diverse settings such as ICU, emergency medicine, behavioral health units, or pediatrics to deliver comprehensive care. Engage with patients with special needs including those with developmental disabilities or dementia, providing specialized memory care or Alzheimer's care as required. Requirements Valid CNA certification from an accredited program with current licensure in the state of employment. Proven experience working in clinical environments such as hospitals (including Level I Trauma Centers), nursing homes, or outpatient clinics. Familiarity with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate medical documentation. Knowledge of physiology and anatomy to support patient assessments and basic life support procedures. Experience working with individuals with disabilities or behavioral health needs is highly desirable. Ability to perform physically demanding tasks including lifting using Hoyer lifts and assisting with transfers safely.
