Keystone Post Acute is a 65-bed community that offers exceptional skilled nursing care for seniors recovering from surgery, rehabilitating from a specific injury or trauma, or those in need of high acuity nursing care. Our highly trained team of nurses are on-site 24 hours a day, 7 days a week, providing general and specialized care to ensure a swift and complete recovery. Compensation based on experience Open Positions 3 Full-Time AM Shift (6:00 AM - 2:15 PM) 2 Full-Time PM Shift (2:00 PM - 10:15 PM) Responsibilities Assist patients with daily activities such as bathing, dressing, grooming, and eating Monitor and record vital signs, including temperature, blood pressure, and pulse Provide companionship and emotional support to patients Assist with mobility and transfers using equipment such as Hoyer lifts Maintain cleanliness and safety of patient areas Document patient care activities accurately in electronic health record (EHR) systems Collaborate with nursing staff to implement care plans tailored to individual patient needs Support patients with Alzheimer's care, dementia care, memory care, and other specialized needs Ensure compliance with HIPAA regulations and maintain confidentiality of patient information Qualifications Valid certification as a Nursing Assistant in the state of practice Experience in critical care, long-term care, assisted living, or nursing home settings preferred Familiarity with EMR systems such as PCC (Point Click Care) is advantageous Knowledge of medical terminology, anatomy, physiology, and basic life support techniques Experience working with individuals with developmental disabilities or in hospice care is a plus Strong communication skills and the ability to work collaboratively within a healthcare team Compassionate demeanor with a commitment to providing high-quality patient service If you are passionate about caregiving and dedicated to enrich the lives of others through compassionate care, we encourage you to apply for this rewarding position.
