$30.00/hour Join Our Team at Oakwood Gardens Care Center! Oakwood Gardens is seeking a Full-Time MDS Assistant to support our clinical team with care coordination and resident assessments. This is an excellent opportunity for an experienced LVN or RN with strong knowledge of MDS processes, Medicare/Medicaid guidelines, and skilled nursing documentation.

Key Responsibilities:

Assist with the coordination and accuracy of MDS assessments and care planning Ensure timely scheduling and submission of assessments per regulatory standards Monitor changes in resident status and communicate with interdisciplinary team Help maintain compliance with Medicare/Medicaid documentation requirements Participate in care plan meetings, audits, and facility surveys as needed Provide nursing support and assist in direct resident care when necessary

Qualifications:

Current California LVN or RN license in good standing Strong understanding of MDS process, care planning, and RAI guidelines Prior experience in a skilled nursing or long-term care facility required Proficient with electronic health records (PCC preferred) Detail-oriented with strong communication and organizational skills
