Per Diem / Per Visit Compensation Registered Nurse (RN): $60•80 per visit Licensed Vocational Nurse (LVN): $40•50 per visit About HRM Home Health HRM Home Health is a patient-centered home health agency committed to delivering exceptional care to patients in their homes throughout Fresno County. We are seeking compassionate, reliable, and motivated Field RNs and LVNs to join our growing team. Registered Nurse (RN) Responsibilities Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments. Perform comprehensive patient assessments and develop individualized care plans. Coordinate care with physicians, therapists, and other healthcare professionals. Provide patient and caregiver education. Ensure accurate and timely documentation in accordance with Medicare and agency requirements. Supervise LVNs and Home Health Aides as needed. Licensed Vocational Nurse (LVN) Responsibilities Provide skilled nursing visits under the supervision of an RN. Administer medications and treatments as ordered by the physician. Perform wound care and monitor patient conditions. Educate patients and caregivers on disease management and treatment plans. Complete accurate clinical documentation. Qualifications RN Requirements Active California RN License. Home Health experience preferred. Strong clinical assessment and communication skills. Reliable transportation, valid driver's license, and current auto insurance. LVN Requirements Active California LVN License. Home Health experience preferred.
