In this vital role, you will provide direct patient care, assist with daily living activities, and support the overall well-being of patients across various care settings. Responsibilities . Monitor and record vital signs, observe patient conditions, and report changes promptly to nursing staff. Ensuring adherence to safety protocols and HIPAA regulations. Maintain accurate medical documentation within Electronic Health Record (EHR) systems, including notes on patient progress and medical records. Assist with specialized care needs including Alzheimer's, dementia, memory care, behavioral health, pain. Follow infection control practices like handwashing technique. Operate medical equipment such as EMR systems, Alpha-stim. Support patients across various settings including inpatient units, outpatient clinics, assisted living facilities, nursing homes, hospice environments, or community-based care programs. Qualifications Valid certification as a Nursing Assistant (CNA) from an accredited program. Willing to drive and visit patients at home. Experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, or home & community care settings. Familiarity with EMR/EHR systems for accurate medical documentation. Knowledge of vital signs measurement, anatomy and physiology basics, infection control practices including aseptic technique. Prior experience with specialized care areas such as Alzheimer's/dementia/memory care or behavioral health is highly desirable. Ability to work effectively with patients and support their physical and emotional needs. Strong communication skills coupled with a compassionate attitude toward patient service and caregiving responsibilities.
