Responsibilities Provide compassionate assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient observations diligently, recording vital signs and noting changes in health status using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Assist with medication administration following prescribed protocols while maintaining HIPAA compliance for patient confidentiality Support clients with specialized needs such as Alzheimer's care, dementia care, hospice support, and memory care strategies Operate medical equipment safely, including Hoyer lifts, catheters, and other assistive devices to ensure patient safety and comfort Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to maintain a clean and safe environment Document medical records accurately and communicate effectively with healthcare teams regarding patient progress or concerns Engage with clients respectfully across all age groups, from pediatric to geriatric populations, ensuring personalized care plans are followed Qualifications Valid CNA certification or equivalent caregiving credentials; prior experience in nursing homes or assisted living preferred—preferred, not required. Strong knowledge of medical terminology and procedures related to home health and long-term care settings Preferred, not required. Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease, or other cognitive impairments Proficiency in using EMR/EHR systems for documentation and patient record management Ability to perform patient observation accurately and respond swiftly to changing health conditions CPR certification required; additional certifications such as hospice care or catheter care are a plus Demonstrated ability to work independently while maintaining a positive attitude and excellent communication skills Prior experience with medical documentation, medication administration, vital signs monitoring, and use of mobility aids such as Hoyer lifts Join us in delivering top-quality home care that empowers clients to live safely and comfortably.
