Hospice CNA with CHHA

MUST HAVE BOTH CNA LICENSE AND CHHA CERTIFICATE- THIS IS A PRN POSITION

Job Summary We are seeking a compassionate and dedicated Hospice Certified Nursing Assistant (CNA) with CHHA certificate to join our team. The Hospice CNA will provide essential support to patients in their homes or facilities, ensuring comfort and dignity during their end-of-life care. This role requires a strong commitment to patient-centered care, empathy, and the ability to work collaboratively with healthcare professionals and family members. Responsibilities Assist patients with daily living activities, including bathing, grooming, dressing, and feeding. Monitor and document vital signs and report any changes in patient conditions to the nursing staff. Provide catheter care and support for patients with memory care needs. Utilize aseptic techniques when performing personal care tasks. Assist with mobility and transfers to ensure patient safety. Maintain a clean and safe environment for patients by following infection control protocols. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Support families by providing emotional support and education regarding patient care. Experience Previous experience in hospice or palliative care settings is preferred. Familiarity with outpatient services, acute care, emergency medicine, or infectious disease care is beneficial. Experience using electronic health records systems such as Cerner is a plus. Knowledge of Level II Trauma Center protocols and hospital medicine practices is advantageous. A valid CNA certification is required. Pay:

$30.00 - $50.00 per hour
