Job Description

Do you enjoy building meaningful connections while providing hands-on care and support to older adults? Frasier is seeking a talented, passionate, and experienced CNA to join our team. In our Summit Care Center, you'll deliver high-quality hands-on care and partner closely with nurses and an interdisciplinary team to support resident dignity, comfort, and safety.

Pay Range :
$20.80 - $23.53 per hour PLUS shift differentials: Evening shift differential +$1.75/hour Overnight shift differential +$2.75/hour Weekend shift differential +$1.75/hour

WHAT YOU'LL DO
Provide compassionate, person ‑ centered care that supports resident dignity, comfort, and independence, including assistance with ADLs such as bathing, dressing, grooming, toileting, oral care, and feeding support. Support resident comfort, mobility, and safety through timely response to call lights, repositioning, ambulation assistance, safe transfers, and use of approved assistive devices and equipment (Hoyer lift, sit‑to‑stand, Sara Stedy, gait belt). Observe and communicate resident needs by recognizing early signs of health or behavioral changes and promptly sharing updates with nursing staff. Provide dementia ‑ supportive care using appropriate communication, redirection, reassurance, and calm engagement while supporting familiar routines and meaningful daily activities. Document and communicate care accurately by completing timely entries in PointClickCare (PCC), participating in shift report, and following infection‑prevention, safety, and mandatory reporting requirements in accordance with Colorado law and organizational policy.
WHAT YOU KNOW AND HAVE EXPERIENCED
Active Colorado Certified Nursing Assistant (CNA) certification. CPR/BLS certification preferred (or willingness to obtain). Ability to safely assist with transfers, mobility, and use of assistive devices. A compassionate, respectful approach to care and a genuine enjoyment of working with seniors. Strong observation, communication, and teamwork skills in a collaborative care environment. Understanding of resident safety, infection prevention, and basic documentation practices. Availability to work a schedule that may include weekends and holidays. Experience in long‑term care or supporting older adults, including those with dementia, is helpful but not required.
