Certified Nursing Assistant (CNA) - Home Health SummitWest Care | Grand Junction, CO Full-Time or Part-Time | Flexible Scheduling $17.00-$22.00/hour (DOE) SummitWest Care is a community-based nonprofit home health agency serving Mesa County. Our mission is to help individuals remain safely and comfortably in their homes while maintaining dignity, independence, and quality of life. We are seeking compassionate Certified Nursing Assistants (CNAs) who are committed to providing exceptional care and building meaningful relationships with clients and families. This position allows you to work one-on-one with clients in their homes, supporting their health, comfort, and daily living needs. What You'll Do Personal CareClient Support Assist clients with activities of daily living, including bathing, dressing, grooming, oral hygiene, toileting, and mobility assistance Support clients in maintaining comfort, dignity, and independence within their homes Follow individualized care plans and client-specific instructions Observe and report changes in client condition, behavior, or environment HealthSafety Monitoring Monitor and document client status, including vital signs when appropriate Assist with treatments and tasks within scope of practice Support infection prevention and safety procedures Communicate concerns promptly to nursing staff and supervisors Home Environment Support Assist with light housekeeping duties such as laundry, changing linens, vacuuming, cleaning, organizing living spaces, and maintaining a safe home environment Ensure client care areas remain clean, comfortable, and functional CommunicationDocumentation Complete required documentation accurately and in a timely manner Maintain clear communication with clients, families, nurses, and other members of the care team Participate in staff meetings, case conferences, and ongoing training as required Professional Growth Maintain active CNA certification Complete required continuing education and training Stay current with agency policies, state regulations, and best practices in home health care Home health care is personal. Compassion, reliability, professionalism, and strong communication skills are essential in this role. What We're Looking For Active Colorado CNA certification High school diploma or GED Strong interpersonal and communication skills Ability to work independently while remaining connected to a care team Reliable attendance and dependable transportation Ability to maintain confidentiality and professional boundaries Strong observational and documentation skills Commitment to providing client-centered care Preferred but Not Required Previous home health, long-term care, hospice, or healthcare experience Experience working with seniors or individuals with disabilities Familiarity with electronic documentation systems Physical Requirements This position requires frequent standing, walking, bending, reaching, lifting, transferring, and assisting clients in the home environment.
Candidates must be able to:
