Job Overview We are seeking compassionate and dedicated caregiving professionals to provide exceptional in-home and community-based care to individuals across diverse settings. The ideal candidates will possess a strong commitment to patient-centered care, demonstrating proficiency in assisting with activities of daily living (ADLs), and health monitoring. Candidates should be motivated, detail-oriented, and eager to support clients in maintaining independence and quality of life. Duties Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring timely communication of changes to healthcare teams. Provide specialized care for clients with dementia or Alzheimer's disease, including memory care strategies and behavioral management. Document medical records and care notes using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems efficiently and accurately. Assist with medical procedures such as catheter care, wound dressing changes, and hospice support as needed. Prepare nutritious meals tailored to dietary restrictions and assist with feeding when required; perform light housekeeping tasks including cleaning and laundry to promote a safe environment. Collaborate effectively with healthcare professionals in assisted living facilities, nursing homes, or home health settings to ensure comprehensive patient care. Engage clients in social activities and provide emotional support to foster a positive environment aligned with their preferences. Qualifications Valid certification as a Home Health Aide (HHA), Certified Nursing Assistant (CNA), or equivalent caregiving credential. Prior experience working in nursing homes, assisted living facilities, or home health environments is highly desirable. Demonstrated ability to observe patients carefully and respond promptly to their needs. Knowledge of medical terminology relevant to caregiving tasks and patient documentation standards. Proficiency in using EMR/EHR systems for accurate record-keeping and reporting. Certification in CPR is required; additional training in dementia care, hospice support, or Alzheimer's care is advantageous. Experience working with individuals with developmental disabilities or disabilities is preferred. Strong communication skills coupled with a compassionate approach to patient care; ability to work independently or as part of a team. $17.00 - $20.00 per hour
