Duties Provide direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor and document patient vital signs and health status using Electronic Health Record (EHR) systems Assist with medication administration following proper protocols and medical documentation standards Support clients with complex needs including catheter care, hospice support, Alzheimer's and dementia care, and working with individuals with developmental disabilities Operate medical equipment such as Hoyer lifts and perform safety procedures for patient transfers Maintain HIPAA compliance by safeguarding patient privacy and confidentiality at all times Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment Requirements Valid CNA certification with current licensure in the state of employment Proven experience in home health care or long-term care settings such as nursing homes or assisted living facilities Strong knowledge of medical terminology, vital signs measurement, medication administration, and infection control practices Experience working with individuals with disabilities or memory-related conditions like Alzheimer's disease or dementia Ability to operate EMR/EHR systems accurately for medical documentation and record keeping CPR certification is required; additional certifications such as hospice care or working with developmental disabilities are a plus Excellent communication skills to effectively collaborate with clients, families, and healthcare teams This role is ideal for dedicated caregivers eager to provide compassionate support in a dynamic home care environment.
