Home Health Aide (HHA/CNA)

Do:

As a Home Health Aide (HHA/CNA), you ll be a vital part of the care team-supporting patients with personal care needs while promoting dignity and independence.

Key responsibilities include:

Assisting with personal care, bathing, and hands-on care in accordance with the patient's plan of care Accurately and promptly documenting each visit per agency policy Notifying the supervising RN of any changes in the patient s condition •

Note:

This role involves multiple short visits throughout the day-not shift work or extended 12-hour cases.

Qualifications:

Completion of a Home Health Aide training program and competency evaluation meeting Medicare s Conditions of Participation Valid driver s license, reliable transportation, and auto insurance (required) Ability to travel to patients homes throughout the day Previous home health experience is a plus, but not required Why Concierge Home Care?
