Job Description

HHAs, CNAs and Companions needed for work in private homes around the Port St. Lucie/Stuart Areas for 4 hours shifts. (8am-12pm; 12pm-4pm and 1pm-5pm shifts needed).. Clients need assistance with all ADL's.
SHIFTS WANTED
!!!!! 8am-12pm 12pm-4pm 1pm-5pm Long term position for a CNA, HHA, and Companions wanting to help. Must Drive. Competitive rate of pay - $16-$18 an hour. Kind loving client looking to stay independent in their home.

DRIVERS LICENSE
SOCIAL SECURITY CARD
PERMANENT RESIDENT CARD
(if not a citizen) Front and Back copy 
AUTO INSURANCE
( in your own name) 

PAST EMPLOYMENT REFERENCES
CNA/LPN/RN LICENSE
HHA CERTIFICATE
(showing 40 hrs training at a licensed school) 
CURRENT CPR TRAINING
(MUST be from American Heart Association, American Red Cross and American Safety & Health Institute) ONLY 
HIV/AIDS TRAINING
(must be for 1 hours or more) 
ASSISTANCE WITH SELF ADMINISTRATION OF MEDICATION TRAINING
(must be for 2hrs or more) 
PHYSICAL EXAM
(physicals from chiropractors not accepted) -Must be dated within the last 6 months 
PROFESSIONAL LIABILITY INSURANCE
(Must be $500,000 per occurrence, with an aggregate of $1,000,000 total) 
LEVEL II BACKGROUND SCREENING
