Job Overview Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) / Home Health Aide (HHA), where your dedication and skills will make a meaningful difference in the lives of individuals needing personalized care. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility to promote independence and comfort Monitor and observe patient conditions, including vital signs and overall health status, documenting changes accurately in Electronic Health Record (EHR) systems Support medication administration following proper protocols and medical terminology understanding, ensuring safety and compliance with HIPAA regulations Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and Alzheimer's-specific activities Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely while maintaining patient dignity Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a clean and safe environment in accordance with home & community care standards Collaborate with healthcare teams on medical documentation, hospice care plans, and patient progress reports to ensure comprehensive care delivery Requirements Valid certification as a Nursing Assistant (CNA) or Home Health Aide (HHA), with current CPR certification required Prior experience in nursing homes, assisted living facilities, or home health environments preferred; experience with long-term care is highly valued Knowledge of medical terminology, medication administration procedures, and vital signs monitoring Experience working with individuals with developmental disabilities or disabilities is a plus; familiarity with dementia/dementia care techniques is essential Ability to operate EMR/EHR systems efficiently for documentation purposes Strong communication skills to effectively interact with patients, families, and healthcare teams while maintaining HIPAA compliance Physical stamina to assist with patient mobility using Hoyer lifts or similar equipment; ability to lift safely is necessary Compassionate attitude focused on patient care, caregiving excellence, and creating a supportive environment for seniors or individuals needing specialized assistance Join us in delivering exceptional home health services that empower individuals to live comfortably and confidently.
