Responsibilities Deliver exceptional patient observation by monitoring health status and recognizing changes in condition. Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Administer medications and perform basic medical procedures such as catheter care, vital signs measurement, and Hoyer lift transfers. Maintain accurate medical documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with HIPAA regulations. Support patients with specialized needs such as dementia care, Alzheimer's care, hospice support, and working with individuals with developmental disabilities or disabilities. Prepare nutritious meals, assist with light cleaning and laundry to promote a safe and comfortable environment. Document patient progress thoroughly and communicate effectively with healthcare teams regarding medical records and patient care updates. Qualifications Valid CNA certification or HHA certification required; prior experience in nursing homes, assisted living facilities, or home health preferred. Strong knowledge of medical terminology and caregiving best practices. Experience working with seniors, individuals with disabilities, or those requiring memory care is highly valued. Ability to work independently in home settings or long-term care environments while maintaining professionalism and compassion. CPR certification is required; additional certifications such as medication administration or hospice care are advantageous. Familiarity with EMR/EHR systems for accurate documentation; excellent communication skills are essential.
