This vital role offers an energetic and rewarding opportunity to provide compassionate, high-quality patient care in a supportive environment. You will be an essential part of our team, ensuring the comfort, safety, and well-being of residents with diverse needs, including those requiring memory care, dementia support, and complex medical attention. Your proactive approach and attentive care will make a meaningful difference in the lives of those you serve. Duties Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates, maintaining detailed medical documentation within electronic health record (EHR) systems such as WellSky. Support patients with specialized needs such as Alzheimer's disease, dementia, or behavioral health conditions by implementing individualized care plans. Assist with medication administration under supervision, ensuring proper handling and adherence to HIPAA regulations for patient privacy. Maintain a safe environment by practicing aseptic techniques during procedures like catheter care and infection control protocols for infectious disease management. Collaborate with interdisciplinary teams in acute care settings such as ICU or Level I Trauma Centers to provide seamless patient service across various departments including emergency medicine and post-acute care. Use medical terminology accurately to document patient progress and communicate effectively with healthcare professionals. Requirements Current certification as a CNA with active state licensure or registration. Prior experience working in long-term care facilities such as nursing homes or assisted living communities is highly preferred. Demonstrated experience with EMR/EHR systems likeWellSky for accurate medical documentation. Knowledge of physiology, anatomy, and basic life support (BLS) procedures necessary for patient safety and emergency response. Experience in diverse clinical environments including hospitals or home care & community care settings is desirable. Ability to work effectively with individuals with disabilities or developmental challenges and provide compassionate senior or pediatric care when needed. Strong communication skills coupled with a proactive attitude toward caregiving and teamwork in fast-paced settings such as urgent or acute care environments.
