As a key member of the Interdisciplinary Group Team, you will work closely with a supervising Registered Nurse and play an essential role in supporting patients during a meaningful stage of life.   • Provide personal care based on assigned patient needs
  • Communicate respectfully and effectively with patients and families
  • Ensure patient safety and proper use of equipment
  • Assist with bathing, grooming, oral care, and linen changes
  • Help with dressing, feeding, transfers, ambulation, and exercises
  • Prepare meals and perform light housekeeping as needed
  • Take and record vital signs
  • Monitor and report changes in patient condition
  • Support caregivers and offer respite when appropriate
  • Maintain clean, orderly patient environments
  • Follow documentation standards and participate in quality improvement activities
  • Uphold dignity and respect in every interaction Requirements
  • Active Certified Nursing Assistant (CNA) certification
  • Minimum 75 hours of CNA training
Preferred:
One year of experience in hospice, home health, or medical/surgical care
  • Valid driver's license and auto insurance
  • Background check upon hire and annually
  • Current CPR certification Ideal Skills and Qualities
  • Ability to follow written instructions
  • Understanding of hospice philosophy
  • Ability to work independently
  • Excellent communication and organizational skills
  • Patience, tact, and professionalism
  • Comfort working with end-of-life care
