You will deliver essential support with dignity and respect, ensuring each client's comfort, safety, and well-being while fostering independence. Responsibilities Provide direct patient care by assisting with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within electronic health record (EHR) systems. Support clients with medication administration, catheter care, hospice needs, and Alzheimer's or dementia care to promote safety and comfort. Assist with medical procedures such as Hoyer lift transfers, wound care, and memory care strategies tailored to individual needs. Maintain a clean and safe environment by performing light housekeeping tasks like laundry, meal preparation, and general cleaning to ensure a hygienic living space. Document all patient interactions accurately while adhering to HIPAA regulations to protect client confidentiality. Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and ensure seamless service delivery. Requirements Valid CNA certification or equivalent caregiving credentials; prior experience in assisted living, nursing homes or home health preferred. Knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation. Experience working with individuals with developmental disabilities, Alzheimer's disease or dementia is highly desirable. Ability to perform patient observation, vital signs measurement, medication administration and use of assistive devices such as Hoyer lifts. CPR certification required; additional training in hospice care or catheter management is a plus. Strong communication skills coupled with compassion and patience; ability to work independently in a home setting. Flexibility for live-in employment opportunities or PRN shifts based on client needs.
