The ideal candidates will possess a strong commitment to enhancing the quality of life for individuals with diverse needs, including those with developmental disabilities, dementia, or requiring hospice care. Responsibilities Observe and monitor patients' conditions, including vital signs, medication effects, and behavioral changes, ensuring timely reporting to healthcare teams. Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Administer medications accurately following medical documentation protocols while maintaining compliance with HIPAA regulations to ensure patient confidentiality. Support patients with specialized care needs, including Alzheimer's care, dementia care, hospice support, catheter care, and memory care routines. Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions, medical records, and care activities precisely. Prepare nutritious meals and assist with meal planning in accordance with dietary restrictions and individual preferences. Maintain a clean and safe environment by performing light housekeeping tasks such as laundry and general cleaning to promote hygiene and comfort. Utilize medical terminology effectively during patient interactions and documentation to ensure clear communication among healthcare providers. Work collaboratively within a team to deliver comprehensive home & community care services tailored to each patient's needs. Demonstrate proficiency in emergency procedures such as CPR and respond promptly to patient emergencies or changes in condition. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly desirable. Experience caring for individuals with developmental disabilities or neurological conditions such as Alzheimer's disease is preferred. Familiarity with medical documentation systems like EHR or EMR platforms is essential for accurate record-keeping. Knowledge of medical terminology related to long-term care, hospice services, and dementia management enhances caregiving effectiveness. Certification in CPR is required; additional certifications such as medication administration or hospice care training are advantageous. Demonstrated ability to work compassionately with diverse populations while maintaining professionalism and confidentiality. Previous experience handling medical equipment like Hoyer lifts or performing catheter care will be considered a strong asset.
