JOB SUMMARY
: Responsible for providing compassionate personal care and homemaker services for patients in the inpatient setting and maintaining a safe and healthy environment as directed by the IDT plan of care.

RN Case Manager
QUALIFICATIONS
Current Certified Georgia Nurse's Aide certificate Current CPR certification Valid driver's license and reliable transportation, and evidence of insurance coverage as required by company policy. Ability to communicate effectively with a strong commitment to customer service and patient/family satisfaction. Ability to complete accurate, real-time electronic charting.
JOB CONDITIONS AND WORK DEMANDS
  • Requires minimal lifting of 50 lbs occasionally and up to 10 pounds of force constantly to move objects.
  • Periods of sitting with frequent kneeling, standing, stooping, pushing, pulling and walking, as well as communication in person.
  • Both inside and outside environmental conditions.
  • Work 12 hours shifts which may include weekends, evenings, and nights.

  • Exposure to fumes, odors, dust, poor ventilation, or smoking
RESPONSIBILITIES
: (Included but not limited to):
  • Provide quality care to residents in an environment which promotes their rights, dignity, freedom of choice, and their individuality.
  • Follow the written Plan of Care as directed by the RN.
  • Observe, report, and document of patient status and the care or service furnished.
  • Observe deviations from the usual physical and mental conditions, report to Primary RN or RN Supervisor.
  • Communicate with and support patient and family.
  • Read and record blood pressure, temperature, pulse, and respiration as directed by the plan of care.
  • Utilize basic infection control procedures.
  • Recognize basic elements of body functioning and changes in body function that must be reported to an aide's supervisor.
  • Maintain a clean, safe, and healthy patient environment.
  • Change bed linens as directed by the plan of care.
  • Perform laundry duties as assigned following established infection control procedures.
  • Recognize emergencies and utilize knowledge of emergency procedures.
  • Show respect for the patient, the patient ' s privacy, and the patient ' s property.
  • Utilize appropriate and safe techniques in personal hygiene and grooming that include bed bath; sponge, tub, or shower bath; shampooing in the sink, tub, or bed; shaving, nail and skin care; oral hygiene; and toileting and elimination as directed by the plan of care.
  • Utilize safe transfer techniques and ambulation as directed by the plan of care.
  • Aide in normal range of motion and positioning after demonstration by RN or therapist as directed by the plan of care.
  • Monitor and report nutrition and fluid intake as directed by the plan of care.
  • Prepare and assist with eating as directed by the plan of care.
  • Perform light housekeeping duties as assigned following established infection control procedures.
  • Any other task that the hospice may choose to have the personal care aide perform, as authorized by law.
  • Document visit record electronically within the assigned shift.
  • Maintain and respect the confidentiality of medical records and information concerning patients.
  • Responsible for submitting patient charges, mileage, and supplies to office staff.
  • Participate in IDT meetings as directed.
  • Cover weekend/holiday schedule as assigned.
  • Perform other duties as assigned.
