What Youll DoNurtures residents, clients, and/or members, ensuring the delivery of top-notch care under supervision of designated departmental leadership and/or other medical professionals as appropriate. Location - 15 Craigside (Assisted Living Clinic)The following shifts are available:
Full-Time (Work 35 to 40 hours per week) 6:00 am - 2:00 pm, Days VariesPay Range$17.42 to $20.75 HourlyResponsibilitiesAnswers call lights to determine residents needs.
Assists residents with activities of daily living (ADLs) such as:
Bathing;Eating; which includes set-up, serving and collecting food plates and utensils,Feeding residents who require assistance; Dressing; Oral hygiene; and Grooming as needed. Provides assistance with toileting and provides appropriate peri-care for incontinent residents. Assists residents to turn, reposition, and transfer, which may be performed by the NA without assistance, with a two-person assistance, or by using a lift machine in accordance with the service plan. Transports residents with ambulation to and from their room to appropriate areas for activities and meals. Takes and records ADLs and chargeable time/unitsVitals (temperature, blood pressure, pulse, weight, and respiration rates)Intake and output of food and fluidTransfers (one or two person) and ambulation statusMood/behaviors when resistant to carePain level assessment (if applicable)Provides companion services according to service plan.
Observes and reports to Licensed Staff all changes in condition (including significant changes), behavior or whereabouts that may require intervention.
Monitors and supervises residents as assigned.
Adheres to assigned work schedules and accurately uses the time and attendance system.
Participates in Continuous Quality Improvement and/or Quality Assurance Programs.
Interacts with residents in accordance with AFCs mission, core values, mindsets and guideposts.
Exercises caution and good safety judgment in uncertain conditions or situations, and report and/or correct hazards, potential or actual.
Ensures that all residents rights and privacy are protected and enforced, and establishes and maintains a close liaison with families. Attends mandatory in-service programs.
Complies with annual employee health screening in accordance with policies and procedures.
Participates and assists during emergency drills and situations such as fire, fire alarms, hurricanes, and other possible emergencies as outlined in the Disaster Response Playbook and/or the disaster policy.

Education/ExperienceHigh school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
Certifications, Licenses, and RegistrationsCurrent State of Hawaii Certified Nurse Aide certificate. Current First Aid and CPR certificate. Computer Skills Proficiency in using a computer, which may include using such software as Vision or Microsoft Office Suite (Excel, Outlook, PowerPoint, Word) and other applicable software applications. Physical DemandsWhile performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to stand, walk, sit, and taste or smell. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl; push and pull. The employee must frequently lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
