Responsibilities Assist residents with daily living activities such as bathing, dressing, grooming. Monitor and record vital signs, including temperature, pulse, and blood pressure and weights. Assist in the mobility of residents by lifting and transferring them safely. Maintain a clean and safe environment for residents. Inform the Charge/Staff Nurse of any changes in the resident's condition so that appropriate information can be entered on the resident's care plan. Collaborate with nursing staff to implement patient care plans effectively. Document patient progress and report any changes in condition to the nursing team. Other duties as assigned by Nursing Coordinator or Director of Supportive Living Skills Strong knowledge of physiology and human anatomy. Experience or training in long-term care settings preferred. Familiarity with systems used in healthcare for patient management is advantageous. Ability to lift patients safely while ensuring their comfort and dignity. Excellent communication skills for interacting with patients, families, and healthcare professionals. Compassionate approach to patient care with an understanding of the unique needs of individuals with dementia or behavioral health challenges. Willingness to learn and adapt in fast-paced environments such as teaching hospitals or critical care units.
