What You'll Do As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families.Your responsibilities include:

What You Bring Active CNA License or Certification in the state of employment Possess and maintains current CPR certification One (1) + year Hospice, home health or related organization experience preferred Comfort and sensitivity working with terminally ill patients and their families A calm, dependable presence under emotionally demanding circumstances Strong attention to detail in observation and documentation A valid driver's license and willingness to travel locally for patient visits Certified Nursing Assistant (CNA)- Hospice Care 3.2 3.2 out of 5 stars Monticello, IL $19
