Job Description
Overview Immediately needed: Visiting Certified Nursing Assistant (CNA)
Areas:
Melrose Park and Summit, IL and the greater Chicago Areas Your compassionate care and attention to detail will make a meaningful difference in patients' lives. This vital role involves providing hands-on assistance, ensuring patient comfort. You will be at the forefront of delivering high-quality patient care with enthusiasm and professionalism. Responsibilities Responsibilities of the home health aide include, but are not limited to, the following: 1.Providing personal care including:
Baths, Back rubs, Positioning, Oral hygiene, Hair care, Changing bed linen, Dressing, Skin care to prevent breakdown. 2. Assisting the patient with toileting activities. 3. Keeping patient's living area clean and orderly, as appropriate. 4. Preparing and serving nutritious meals. 5. Assisting in feeding the patient, if necessary. 6. Performance of simple procedures as an extension of therapy or nursing services such as taking and recording oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency). 7. Assisting in ambulation and exercise according to the plan of care. 8. Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency). 9. Assisting patient in the self-administration of medication. 10. Household services essential to health care at home. 11. Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.). 12. Reporting on patient's condition and significant changes to the registered nurse or appropriate therapist. 13. Adhering to the agency's documentation and care procedures and standards of personal and professional conduct. Experience Completed a nurse aide program or compentency evaluation parogram and Certified by the State of IL Prior experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, or home care is highly valued.Knowledge of EMR/EHR systems:
Axxess is preferred for accurate documentation and efficient workflow management. Certification in Basic Life Support (BLS) is required; additional certifications such as phlebotomy or medication administration are a plus. Familiarity with medical terminology, anatomy, physiology, aseptic techniques, and HIPAA regulations ensures compliance and quality care delivery. Demonstrated caregiving skills across inpatient, outpatient, sub-acute care, or home & community settings reflects versatility and commitment to patient well-being.
