This vital role involves assisting individuals with daily activities, ensuring their safety, and promoting their well-being in various care settings. The ideal candidate will possess a strong background in senior care, medical assistance, and working with individuals with disabilities or memory impairments. This position offers an opportunity to make a meaningful difference in the lives of those requiring specialized care, emphasizing professionalism, empathy, and attention to detail. Client needs 2 days a week for 3 hours 6 hrs a week. Responsibilities Assist residents or clients with activities of daily living, including bathing, dressing, grooming, and mobility support. Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using EMR (Electronic Medical Record) or EHR (Electronic Health Record) systems. Administer medications and perform treatments such as tube feeding or catheter care following established protocols and medical terminology guidelines. Support individuals with Alzheimer's disease, dementia, or other memory-related conditions through personalized care plans focused on safety and comfort. Document all patient care activities meticulously while maintaining strict adherence to HIPAA regulations to ensure confidentiality. Collaborate with nursing staff and healthcare professionals to coordinate patient care plans in various settings including assisted living facilities, nursing homes, hospitals, or home care environments. Maintain a safe environment by following infection control procedures and safety standards aligned with healthcare regulations. Engage in ongoing training related to medical documentation, physiology knowledge, anatomy, and emergency response procedures to enhance caregiving skills. Qualifications Proven experience as a CNA or HMK within assisted living facilities, nursing homes, hospitals, or home care environments. HS Diploma or refrence letter transcript. Familiarity with medical documentation systems such as Epic or Cerner is preferred. Knowledge of medical terminology, vital signs monitoring, medication administration, and infection control practices. Experience working with individuals with developmental disabilities or special needs is highly desirable. Strong understanding of anatomy and physiology relevant to patient care procedures such as catheterization and tube feeding. Ability to work effectively with diverse populations including seniors and individuals requiring hospice or dementia care. Certification as a CNA or equivalent credential recognized by local health authorities; ongoing education in medical office procedures is advantageous. Excellent communication skills coupled with compassion and patience essential for providing high-quality caregiving services.
