Home Health Aide & CNA

This caregiver role is designed for a certified Home Health Aide (HHA) or Certified Nursing Assistant (CNA) to deliver exceptional in-home care to our clients. The ideal candidate will possess a strong background in patient care , with expertise in assisting with activities of daily living (ADLs) and providing emotional support. This role offers an opportunity to make a meaningful difference in the lives of homebound individuals with skilled care needs. The organization emphasizes the importance of qualified caregivers and personalized services to ensure peace of mind for patients and their families. Provide assistance with activities of daily living , including bathing, dressing, grooming, and mobility support. Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to ensure patient health as directed by an RN. Support patients with medical needs such as catheter care, ostomy care, Alzheimer's/dementia care, and memory care. Operate medical equipment including Hoyer lifts and EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation. Assist with meal preparation, feeding, and ADLs to increase patient safety in the home environment. Collaborate effectively with healthcare teams to ensure comprehensive patient care plans are followed accurately. Valid certification as a CNA or Home Health Aide with current licensing in Indiana. Prior experience working in assisted living facilities, nursing homes, or home health settings preferred. Knowledge of medical terminology and proficiency in using EMR/EHR systems for documentation. Experience caring for individuals with developmental disabilities, dementia, or Alzheimer's disease is highly desirable. Strong patient observation skills combined with the ability to respond quickly to changing needs under the supervision of an RN. CPR certification is required; additional certifications such as hospice or dementia care training are a plus. Ability to work independently while maintaining excellent communication skills within a team environment.
