  • Responsibilities:
  • Provide technical assistance and support to field staff, ensuring they have the necessary resources and knowledge to deliver high-quality care
  • Conduct on-site visits to clients and caregivers to assess the quality of care and provide guidance and feedback
  • Develop and maintain relationships with clients, caregivers, and healthcare professionals to ensure a seamless delivery of care
  • Monitor and report on client progress, identifying areas of improvement and implementing strategies to enhance care
  • Collaborate with the clinical team to develop and implement personalized care plans that meet the unique needs of each client
  • Conduct training sessions and provide ongoing education to caregivers to ensure they have the necessary skills to deliver high-quality care
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellence in home care
  • Requirements:
  • Certified Nursing Assistant (CNA) certification
  • or equivalent
  • High school diploma or equivalent
  • required; some college or Bachelor's degree in a related field preferred
  • Minimum 1 year of experience
  • in a healthcare setting, preferably in home care or a related field
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, caregivers, and healthcare professionals
  • Ability to travel to client homes and conduct on-site visits, with a valid driver's license and reliable transportation
  • Strong problem-solving and analytical skills, with the ability to think critically and make sound decisions
  • Ability to maintain confidentiality and adhere to HIPAA regulations
