The Wellness Nurse works with the Interdisciplinary team, resident and family to assess and determine needs within an Individual Service Plan (ISP) and coordinate service provision as identified in the ISP. The LPN is responsible for working with the Wellness Director, Resident Care Assistants and Resident Care Supervisors to provide optimum care for each individual resident. The LPN is responsible for general supervision of staff as well as disciplinary action when needed. Also during the course of duty the LPN may be called upon to perform various technical nursing skills.
Essential Duties and Responsibilities :
Assisting staff in the provision of direct care to residents affords not only the opportunity to serve as a role model but also provides the LPN the opportunity to assess Resident Care Assistants skills and a better recognition of the supports and constraints under which the staff function. Function as liaison between staff and administration, staff and families, staff and service providers including physicians to promote optimal care for the resident on both a formal and informal basis. Works with the Wellness Director to develop and coordinate the community educational programs including, but not limited to, orientation and in-service for all personnel. With the Wellness Director, directs, evaluates and supervises all resident care and initiates corrective action as necessary. Evaluates potential residents. Reviews residents admitted and implements with the interdepartmental team the Individual Service Plan for each resident. Identifies safety hazards and initiate corrective action as necessary. Completes reports and assignments accurately and adhere to established time schedules. Establishes open lines of communication between resident care assistants and housekeeping and other departments consistently. Interviews, hires and counsels Resident Care Assistants as necessary.
