Job Overview Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) specializing in Home Care! In this vital role, you will provide personalized, high-quality care to individuals in their own homes, promoting independence and enhancing quality of life. Your energetic and caring approach will make a meaningful difference for patients with diverse needs, including those requiring memory care, hospice support, or assistance with daily activities. This position offers a rewarding opportunity to work closely with clients, ensuring their safety, comfort, and well-being through dedicated caregiving. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using Hoyer lifts or other assistive devices. Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status. Support medication administration following proper protocols while maintaining accurate medical documentation and adhering to HIPAA regulations. Provide specialized care for individuals with dementia or Alzheimer's disease by implementing memory care techniques and ensuring a safe environment. Perform patient observation to identify changes in condition or behavior and communicate findings effectively to healthcare professionals. Assist with medical procedures such as catheter care, wound dressing changes, and hospice support as needed. Manage EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems to document patient care activities accurately. Support clients with household tasks including meal preparation, cleaning, laundry, and light housekeeping to promote a safe and comfortable living environment. Use medical terminology correctly when documenting patient information or communicating with healthcare teams. Work with individuals who have developmental disabilities or disabilities requiring specialized attention in home and community settings. Qualifications Valid CNA certification from an accredited program; CPR certification required. Prior experience in nursing homes, assisted living facilities, or home health environments preferred; long-term care experience is a plus. Demonstrated ability in patient observation, medication administration, and dementia or Alzheimer's care. Familiarity with EMR/EHR systems for accurate documentation of patient records. Knowledge of medical terminology and procedures such as catheter care and vital signs monitoring. Experience working with individuals with disabilities or developmental challenges is highly desirable. Strong communication skills and the ability to work independently while following healthcare protocols.
