The Difference You'll Make Provide direct care to residents in accordance with individualized care plans Take and record vital signs accurately and report any changes in condition promptly Monitor residents' physical and emotional well-being and communicate concerns to nursing staff Assist residents with mobility and transfers using approved techniques and equipment (gait belts, mechanical lifts, etc.) Support residents with daily care needs while preserving dignity and independence Respond promptly to call lights and resident requests Foster a respectful, compassionate environment for residents, families, and team members

Qualifications:

Experience Required:
