We are seeking compassionate and dedicated Caregivers who are qualified as PCAs, CNAs, and HHAs, or those who have relevant job-based experience in personal care to join our team. The position will support clients in Plymouth, Minneapolis, Maple Grove, Osseo, Rogers, Mound, Delano, Hamel, Maple Plain, Long Lake and the surrounding communities. Responsibilities Provide companionship and emotional support to clients to promote mental well-being Assist clients with activities of daily living including bathing, dressing, grooming, toileting, personal hygiene and mobility support Support medication reminders and monitor vital signs as directed by healthcare professionals Support clients with meal preparation and feeding assistance when necessary Follow care plans tailored to individual clients needs Maintain a clean and safe environment for clients by providing light housekeeping and adhering to infection control protocols and aseptic techniques Communicate effectively with family members and healthcare teams regarding client status and progress Assist with transportation to medical appointments or community activities when required Qualifications Valid certification as a CNA, HHA, PCA, or relevant experience qualifying the individual to pass a skills-based assessment to perform personal care services in compliance with state and federal regulations. Ability to work independently while following detailed care plans Compassionate attitude with excellent communication skills Employees must have the physical ability, to lift, push, and pull up to 50 pounds Must be reliable, patient-focused, and committed to providing high-quality patient-centered care This position offers an enriching environment for those passionate about healthcare and making a positive impact on others' lives.
