Caregiver/Home Health Aide/CNA

Job Overview AscendCare Home Health Solutions LLC is seeking compassionate and dependable Caregivers/Home Health Aides (HHAs) to join our team and provide high-quality care to clients in their homes. Our caregivers play a vital role in helping individuals maintain dignity and independence while receiving supportive services in the comfort of their familiar surroundings. As a Home Health Aide, you will assist with personal care, light housekeeping, and companionship to clients in need, under the supervision of a Registered Nurse. Key Responsibilities Provide personal care assistance including bathing, grooming, dressing, and toileting Assist with ambulation, transfers, and mobility support Medication administration, reminder, assistance Prepare and serve meals; assist with feeding as needed Perform light housekeeping tasks such as laundry, dishes, and tidying Offer emotional support and companionship Monitor and report changes in client condition to the RN supervisor Follow individualized care plans and document services provided Transportation, errands, shopping

Qualifications:

High school diploma or GED Completion of a Home Health Aide, CNA, or PCA training program Must be at least 18 years of age Ability to pass a criminal background check and TB screening Reliable transportation Strong communication and interpersonal skills Compassionate, patient, and dependable Willingness to learn Respectful of clients, client's homes, and their wishes/rights

Preferred Qualifications:

Previous home care experience Experience working with seniors or individuals with disabilities Culturally sensitive and respectful of client preferences Familiarity with dementia care CPR and First Aide Certified (will train) Experience with medication: administration, reminders, and assistance Experience documenting in Electronic Health Record System (
